It has come to our attention that two employees of our supplier who are under 18 years of age were mistakenly vaccinated with a vaccine manufactured by Moderna, which is intended for 18 years of age or older. We would like to express our sincere apologies for causing great concern. Our industrial physicians and nurses have been checking the health of the two employees over the phone, and there have been no problems with their physical condition to date.

The background of this incident is as follows; On Monday, July 5 2021, Toyota conducted workplace vaccination at Tahara Plant. Although we verified the identity of each individual at the reception desk and also during the doctor's interview, the process of verifying the date of birth and age was not included. For the inoculated individuals, since they looked younger than the other members, their age were confirmed at the reception desk after the inoculation, and it was discovered that they were under 18 years old.

We take this incident very seriously and have implemented the following measures to prevent recurrence at all of our locations, starting with the next day's vaccination at the workplace.

  1. Added "age verification" and "no vaccination under 18 years old" description to the operation manual, then addition of a process to check the age of vaccine recipients at the reception desk
  2. Age is checked again at the doctor's interview.
  3. Informing suppliers in advance that vaccination is not available for those under 18 years old
  4. Adding a signboard at the entrance of vaccination sites stating that vaccination is not available for those under 18 years old

We will do our utmost to prevent the recurrence of such incidents in cooperation with local governments to ensure safe and secure vaccinations.
